The Aefina Partners Co-design Method is based in human-centered co-design: a process where people combine their unique experiences and skills to design the interactions, systems, and spaces that enable thriving patient-team partnerships.
The Aefina Partners Co-design Method offers health systems and medical groups a means to achieve healthcare transformation through promoting partnerships that add value. The partnerships are focused in four areas – experience, outcomes, value and engagement. The Method assures actions are tailored to organizational and individual needs. It enables healthcare organizations to enhance the capacity of leaders, physicians and other team members to develop effective patient relationships while also improving organizational and team performance.
Based on collaborative partnerships, we aid organizations to achieve results in patient partnerships, productive team relationships that restore joy and meaning in work, and effective, respected leadership. Working with leaders in a variety of settings, the Aefina Partners Co-design Method includes internal assessment emphasizing asset identification. The approach builds on organizational assets to tailor strategy design at three levels: patient-practitioner connection, care environment, and organization. Clear strategy leads to an implementation plan; the identification, use and communication of plan metrics and goals; and infrastructure development that sustain results.
